Our Services
At Shaw Family Medical, our goal is to serve the community by providing outstanding healthcare to our patients through our small town attitude of qualified compassion and professional responsiveness. We pride ourselves on the personal relationships we establish with our patients as well as the comprehensive services we provide in the specialty of Family Medicine. Our practice is open to patients of all ages, from 6 weeks old and up.
Our caring staff works together with you to be a partner in the planning of your health care needs. We provide preventive services, health maintenance exams, on-site x-ray, diagnostic testing, family planning, and disease prevention, detection and treatment. We provide women's services by female healthcare providers. Our female Nurse Practitioners provide medical services for Bolivar County and all surrounding areas. At Shaw Family Medical you will find a caring, comfortable environment and dedicated providers that practice medicine with honesty, integrity and compassion.

This form must be downloaded using an Adobe Application or compatible application to complete the form in the fillable format. This form can also be printed and completed in the paper form and sent back to school/given to our staff.
